Step 2. Attach 2 current photos

1) A headshot and
2) A full body image. Note: Please write name, address, email address and phone number on the back of all photos.

Step 3. Mail completed application, including photos, along with a $35 non-refundable entry fee payable to SuperStarz Magazine, LLC to:
